No EMR at all

Wearables & phone health platforms
Healthvox reads from Apple Health and Android Health Connect rather than asking patients to buy particular hardware. Any device whose own app writes to those stores reaches you without a separate integration — which is most of them.
Apple Health (HealthKit)

Android Health Connect
