Most care plans end when the patient leaves the room.
A printed plan or a portal note rarely survives the first busy week.
No daily touchpoint
The plan isn't part of the patient's day, so it fades.
Progress is invisible
Neither patient nor physician can see whether the plan is working until the next visit.
Manual logging
Asking patients to retype readings their devices already captured guarantees drop-off.
Goals, targets and a daily check-in.
Journeys are live in the Healthvox app today.
Goals patients recognize
Losing weight, lowering blood pressure, reversing prediabetes, sleeping better and more — up to three at a time.
Journeys the care team sets
Assign up to three journeys with a target, such as a systolic blood pressure goal. Patients see them marked as set by their care team.
One check-in each morning
A short daily check-in keeps the journey part of the patient's routine.
Readings filled in for them
Values a scale, cuff or glucose meter already sent are completed automatically — no retyping.
Daily goals from day one
Patients set daily goals at signup, so the app is personal from the first morning.
Progress at the visit
Journey progress appears in the pre-visit brief alongside trends and labs.

Frequently asked questions
Can patients choose their own journeys?
Yes. Patients can pick up to three goals themselves, and the care team can assign up to three journeys with targets. Care-team journeys are labeled so patients know who set them.
Which devices fill in check-ins automatically?
Any reading that reaches Healthvox through Apple Health, Health Connect or LibreLinkUp — for example, a connected blood pressure cuff, smart scale or glucose meter.
Do members need to buy specific devices?
No. Healthvox reads from Apple Health on iPhone and Health Connect on Android, so Apple Watch, Samsung Galaxy Watch, Pixel Watch, Fitbit, Garmin, Withings and Omron data arrives through the platform health store. FreeStyle Libre glucose data connects directly through LibreLinkUp.
